胃癌組織Oct4和PI3K的表達和相關(guān)性

目的:探討八聚體結(jié)合轉(zhuǎn)錄因子(Oct4)與磷脂酰肌醇3-激酶(PI3K)在胃癌組織中的表達和意義,并分析二者的相關(guān)性。方法:應(yīng)用免疫組化S-P法檢測60例胃癌組織和25例正常胃組織(距癌組織邊緣10cm以上的胃組織)Oct4與PI3K蛋白的表達情況。結(jié)果:胃癌組織Oct4、PI3K的陽性表達率均明顯高于癌旁正常胃組織(P<0.01);胃癌組織Oct4、PI3K的表達均與分化程度、浸潤深度、淋巴結(jié)轉(zhuǎn)移有關(guān)(P<0.05),與年齡和性別無關(guān)(P>0.05)。胃癌組織Oct4、PI3K的表達呈正相關(guān)關(guān)系(r=0.532,P<0.05)。結(jié)論:Oct4和PI3K可能與胃癌的發(fā)生、發(fā)展及浸潤轉(zhuǎn)移密切相關(guān)。

胃癌在我國的發(fā)病率和死亡率總體呈上升趨勢,因此胃癌的發(fā)生機制成為了研究的熱點。八聚體結(jié)合轉(zhuǎn)錄因子4蛋白(octamer binding factor 4,Oct4)對細胞的正常分化起著決定性作用[1]。PI3K/Akt信號通路的活化可促進細胞增殖,并能促進腫瘤細胞的轉(zhuǎn)移和侵襲[2]。PI3K是PI3K/Akt信號轉(zhuǎn)導(dǎo)通路的始動因子。目前,對于在胃癌發(fā)生、發(fā)展過程中Oct4和PI3K信號轉(zhuǎn)導(dǎo)通路相互作用的研究報道較少。本研究通過檢測正常胃組織與胃癌組織中Oct4與PI3K的表達情況,分析胃癌組織中Oct4與PI3K的相互關(guān)系,以探索胃癌的發(fā)生、發(fā)展機制,以期為臨床治療胃癌提供新的作用靶點。
1.1 一般資料 承德醫(yī)學(xué)院附屬醫(yī)院2013年7月至2014年2月手術(shù)切除胃癌組織60例及距癌組織邊緣10cm以上的正常胃組織25例,術(shù)前均未行化療、放療,所有患者均經(jīng)術(shù)后病理學(xué)檢查證實為胃腺癌,相應(yīng)癌旁組織均未見癌累及。60例胃癌患者中,男45例、女15例,年齡39-80歲,中位年齡56歲;高-中分化24例、低分化36例,癌組織侵及漿膜層48例、未侵及漿膜層12例,無淋巴結(jié)轉(zhuǎn)移25例、有淋巴結(jié)轉(zhuǎn)移35例。所有標本離體后立即置于10%福爾馬林溶液中固定,常規(guī)石蠟包埋、切片。
1.2 免疫組化染色及結(jié)果判定 采用S-P法,兔抗人Oct4單克隆抗體、兔抗人PI3K多克隆抗體,購自博士德生物工程有限公司,工作濃度為1:100。以PBS代替一抗作為陰性對照,用已知陽性切片作為陽性對照。Oct4、PI3K陽性染色呈黃色至棕黃色,主要定位于癌細胞的細胞質(zhì)。在染色均勻的區(qū)域,選取5個高倍鏡視野(×400):⑴按陽性細胞百分率評分(A值):1%-25%為1分,25%-50%為2分,>50%為3分;⑵按染色強度評分(B值):不著色為0分,淺棕黃色為1分,棕黃色為2分,棕褐色為3分。A、B兩項得分相加≤2分為陰性,>2分為陽性。
1.3 統(tǒng)計分析 采用SPSS 17.0統(tǒng)計軟件進行處理,率的比較采用卡方檢驗,相關(guān)分析采用Spearman相關(guān)分析。P<0.05為差異有統(tǒng)計學(xué)意義。
2.1 Oct4、PI3K蛋白的表達情況及二者的相關(guān)性 胃癌組織Oct4蛋白的陽性表達率為71.67%(43/60),PI3K蛋白的陽性表達率為86.67%(52/60),均明顯高于正常胃組織(P<0.01)。胃癌組織中,Oct4蛋白和PI3K蛋白的表達呈正相關(guān)關(guān)系(r=0.532,P<0.05)。見表1-2:

2.2 Oct4、PI3K蛋白表達與胃癌病理特征的關(guān)系 胃癌組織Oct4、PI3K蛋白的表達與患者年齡、性別無關(guān)(P>0.05);與胃癌的分化程度、浸潤深度和有無淋巴結(jié)轉(zhuǎn)移有關(guān)(P<0.05):低分化明顯高于高-中分化、侵及漿膜明顯高于未侵及漿膜、有淋巴結(jié)轉(zhuǎn)移明顯高于無淋巴結(jié)轉(zhuǎn)移。見表3:

Oct4基因是八聚體核苷酸結(jié)合蛋白轉(zhuǎn)錄因子家族中的一員,早期發(fā)現(xiàn)于胚胎干細胞中[3-4]。人的Oct4蛋白在調(diào)節(jié)胚胎發(fā)育、維持胚胎干細胞自我更新能力及定向分化能力方面具有重要作用[5-6]。除在胚胎和生殖細胞癌中表達外,有研究者在乳腺癌MCF7細胞和宮頸癌Hela細胞株等非生殖細胞癌中也發(fā)現(xiàn)了Oct4的表達[7-10]。本研究采用免疫組化技術(shù)對Oct4蛋白在胃癌中的表達情況進行了檢測,發(fā)現(xiàn)胃癌組織Oct4的陽性表達率明顯高于正常胃組織;并且發(fā)現(xiàn),胃癌組織Oct4表達與腫瘤的分化程度、浸潤深度和淋巴結(jié)轉(zhuǎn)移有關(guān):低分化明顯高于高-中分化、侵及漿膜明顯高于未侵及漿膜、有淋巴結(jié)轉(zhuǎn)移明顯高于無淋巴結(jié)轉(zhuǎn)移。因此提示,隨著胃癌惡性程度的增加,Oct4的陽性表達率逐漸升高。
PI3K是PI3K/Akt信號轉(zhuǎn)導(dǎo)通路的始動因子,此信號通路是一條酪氨酸激酶級聯(lián)信號通路。Src、Ras、Rac、Ab1、v-Crk等具有酪氨酸激酶活性的癌性蛋白可與PI3K的調(diào)節(jié)亞基P85結(jié)合而發(fā)生作用,這些癌性蛋白的異?;罨墒筆I3K持續(xù)活化,進而使細胞形態(tài)改變、凋亡受阻,導(dǎo)致細胞轉(zhuǎn)化及過度增殖[11]。本研究顯示,胃癌組織PI3K蛋白的陽性表達率明顯高于正常胃組織,隨著胃癌分化程度高、中→低變化,浸潤深度由淺→深改變,PI3K的陽性表達率明顯升高,且有淋巴結(jié)轉(zhuǎn)移者PI3K的陽性表達率也明顯高于無淋巴結(jié)轉(zhuǎn)移者。提示PI3K的激活與胃癌的發(fā)生發(fā)展和浸潤、轉(zhuǎn)移有關(guān)。
綜上所述,本研究證實了胃癌組織Oct4、PI3K的陽性表達明顯升高,提示二者均參與了胃癌的發(fā)生、發(fā)展及浸潤、轉(zhuǎn)移等生物學(xué)行為,因此,本研究可為胃癌的臨床治療提供新的作用靶點。同時,本研究還發(fā)現(xiàn),胃癌組織Oct4與PI3K的表達呈正相關(guān)關(guān)系,二者的相互作用可能為胃癌的發(fā)病機制提供新的理論依據(jù)。

EXPRESSION AND CORRELATION OF OCT4 AND PI3K IN GASTRIC CARCINOMA
Objective: To investigate the expression and signif icance of octamer-binding transcription factor (Oct4)and phosphatidyl inositol 3-kinase (PI3K) in gastric carcinoma, and analyze the correlations between them. Methods: S-P immunohistochemical stain was used to detect the expression of Oct4 and PI3K in 60 cases gastric carcinoma and 25 casesnormal gastric tissues. Results: The positive expression rate of Oct4 and PI3K in gastric carcinoma was obviously higher than normal gastric tissues (P<0.01). In gastric carcinoma, the expression of Oct4 and PI3K were all related to differentiated degree, infi ltration depth and lymph node metastasis (P<0.05); but not related to age and sex (P<0.05). In gastric carcinoma,the expression of Oct4 was positively correlated with PI3K (r=0.532,P<0.05). Conclusions: Oct4 and PI3K may play important role in development, infi ltration and metastasis of gastric carcinoma.
