【摘要】目的 應(yīng)用二尖瓣環(huán)自動(dòng)追蹤技術(shù)(AMAT)測(cè)量二尖瓣環(huán)收縮期位移,探討AMAT評(píng)價(jià)左室收縮功能的準(zhǔn)確性。方法 選取受檢者60例作為研究對(duì)象,其中LVEF>50%者30例,LVEF<50%者30例。取心尖四腔心,分別采用M-型超聲心動(dòng)圖法(M-型法)和AMAT法測(cè)量二尖瓣環(huán)間隔側(cè)及側(cè)壁處的收縮期位移(MAD-S、MAD-l);并用改良Simpsons法測(cè)定左室射血分?jǐn)?shù)(LVEF)。結(jié)果 AMAT和M-型法測(cè)量的MAD-S、MAD-l無(wú)顯著性差異(P>0.05);并與LVEF相關(guān)性良好(r=0.89和r=0.86)。結(jié)論 AMAT可以簡(jiǎn)便、快速、準(zhǔn)確地測(cè)量二尖瓣環(huán)收縮期位移,為評(píng)價(jià)左室收縮功能提供了一種新方法。

【Abstract】Objective To evaluate the accuracy of automated mitral annular tracking (AMAT ) for assessing left ventricular systolic function through detemination mitral annular []displacement(MAD).Methods Sixty subjects were studied,including 30 subjects with LVEF>50% and 30 subjects with LVEF<50%,.LVEF of all subjects were determined by 2D echocardiography using Simpsons rule. MAD of septal and lateral sites of mitral annulus(MAD-S and MAD-l) from apical four-chamber view were determined by AMAT and M-mode echocardiography.Results MAD-S and MAD-l by AMAT had no significant diffrenences when compared with it measured by M-mode echocardiography(P>0.05),and correlated closely with the LVEF determined by 2D echocardiography using Simpsons rule(r=0.89和r=0.86).Conclusions AMAT can simply、rapidly and accurately detect mitral annular displacement,and provide a new technigue for evaluating left ventricular systolic function.

許多研究已經(jīng)證實(shí),二尖瓣環(huán)收縮期位移可以反映左室收縮功能的變化,而且與左室射血分?jǐn)?shù)(LVEF)之間有良好相關(guān)性[1]。本研究旨在探討AMAT法測(cè)量二尖瓣環(huán)收縮期位移評(píng)價(jià)左心室收縮功能的準(zhǔn)確性。
1 資料與方法
1.1 一般資料
選取受檢者60例作為研究對(duì)象,其中LVEF>50%者30例,LVEF<50%者30例。其中,男35例,女25例,年齡28~59歲,平均(47.8±6.9)歲。
1.2 儀器與方法
使用日本東芝公司的Toshiba Aplio XG超聲診斷儀,應(yīng)用改良Simpsons法測(cè)量LVEF。在心尖四腔心切面下,分別采用AMAT法和M-型法測(cè)量MAD-S、MAD-l。AMAT測(cè)量方法及曲線圖見圖1。以上指標(biāo)均取三個(gè)心動(dòng)周期的平均值。
1.3 統(tǒng)計(jì)學(xué)方法
采用SPSS統(tǒng)計(jì)學(xué)軟件對(duì)數(shù)據(jù)進(jìn)行處理,計(jì)量資料以“x±s”表示,采用t檢驗(yàn)。以P<0.05為差異有統(tǒng)計(jì)學(xué)意義。
2 結(jié) 果
2.1 應(yīng)用AMAT法和M-型法測(cè)量的二尖瓣環(huán)間隔側(cè)及側(cè)壁收縮期位移比較
二尖瓣環(huán)自動(dòng)追蹤技技術(shù)和M-型超聲心動(dòng)圖測(cè)量的MAD-S、MAD-l比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05),見表1。
2.2 二尖瓣環(huán)自動(dòng)追蹤技術(shù)測(cè)量的MAD-S、MAD-l與LVEF具有良好相關(guān)性(r=0.89和r=0.86)
3 討 論
研究證明,左室長(zhǎng)軸收縮活動(dòng)早于左室的短軸收縮活動(dòng),對(duì)維持正常的左室收縮功能起著重要作用[2]。收縮期二尖瓣環(huán)的運(yùn)動(dòng)可以直接反映左室的長(zhǎng)軸收縮運(yùn)動(dòng)[3]。
M-型超聲心動(dòng)圖測(cè)量二尖瓣環(huán)收縮期位移評(píng)價(jià)左室收縮功能是一種通用、可靠的和精確性高的超聲測(cè)量方法,與二維超聲心動(dòng)圖Simpsons法測(cè)量的EF具有良好相關(guān)性,已經(jīng)得到廣泛研究和認(rèn)同[4];而且M-型超聲心動(dòng)圖測(cè)量的二尖瓣環(huán)收縮期位移與核素左室造影及左室造影計(jì)算的LVEF之間有良好的相關(guān)性[5]。AMAT基于二維超聲高幀頻數(shù)字技術(shù),操作簡(jiǎn)單,不受超聲聲束角度的影響,只需在心尖四腔心切面顯示二尖瓣環(huán)間隔側(cè)、側(cè)壁及心尖這3個(gè)取樣點(diǎn),通過(guò)自動(dòng)追蹤左室的心內(nèi)膜在整個(gè)心動(dòng)周期的蛇形運(yùn)動(dòng)情況,計(jì)算出左心室功能及結(jié)構(gòu)的各項(xiàng)指標(biāo)[6]。本研究中AMAT法和M-型法測(cè)量二尖瓣環(huán)收縮期位移無(wú)顯著性差異,表明了AMAT法作為一種新技術(shù)與M-型法測(cè)量二尖瓣環(huán)收縮期位移的一致性。而且AMAT法測(cè)量的二尖瓣環(huán)收縮期位移與LVEF之間具有良好相關(guān)性(r=0.89和r=0.86),為臨床提供了客觀地評(píng)價(jià)左室收縮功能的新方法。
