2016年世界衛(wèi)生組織(WHO)中樞神經(jīng)系統(tǒng)腫瘤分類將間變性星形細(xì)胞瘤,IDH?突變定義為伴異檸檬酸脫氫酶1或2(IDH1或IDH2)基因突變的彌漫性浸潤性星形細(xì)胞瘤,發(fā)生局灶性或散在性間變,增生活躍。間變性星形細(xì)胞瘤可以源自低級別彌漫性星形細(xì)胞瘤,但尤以無低度惡性原始病變的間變性星形細(xì)胞瘤最為常見,可以進(jìn)展為膠質(zhì)母細(xì)胞瘤,IDH?突變。組織學(xué)形態(tài)與彌漫性星形細(xì)胞瘤相同,但細(xì)胞密度、胞核異型性和核分裂象增加(圖1)。局灶性或彌漫性細(xì)胞密度增加是重要診斷標(biāo)準(zhǔn)。胞核多形性更為復(fù)雜,大小、形態(tài)不一,染色質(zhì)密集或分散,核仁明顯、數(shù)目增加。亦可見多核細(xì)胞和豐富的核分裂象,無微血管增生(多層血管)和壞死。免疫組織化學(xué)染色,腫瘤細(xì)胞胞質(zhì)彌漫性表達(dá)膠質(zhì)纖維酸性蛋白(GFAP,圖2),胞核彌漫性表達(dá)P53,胞質(zhì)高表達(dá)和胞核低表達(dá)R132H?突變的IDH1,通常不表達(dá)α地中海貧血X連鎖智力低下綜合征基因(ATRX)。
