動態(tài)監(jiān)測C反應(yīng)蛋白在AECOPD機械通氣患者中的臨床價值

摘要目的:探討動態(tài)監(jiān)測C反應(yīng)蛋白(C-reactive protein,CRP)在行機械通氣的慢性阻塞性肺疾病急性發(fā)作期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)中的臨床價值。方法: 檢測36例行機械通氣的AECOPD患者入院后第 1、3天的CRP,記錄入院時的動脈血氣分析結(jié)果, 采用急性生理學(xué)與慢性健康狀況評分系統(tǒng)Ⅱ(APACHE Ⅱ)進行評分。根據(jù)患者預(yù)后情況分為生存組和病死組并進行比較。結(jié)果:兩組性別、年齡差異無統(tǒng)計學(xué)意義(P>0.05)。兩組入院第1天CRP差異無統(tǒng)計學(xué)意義(P>0.05),而第3天時CRP差異有統(tǒng)計學(xué)意義(P<0.05)。兩組的APACHEⅡ評分差異無統(tǒng)計學(xué)意義(P>0.05)。兩組入院時PaCO2差異有統(tǒng)計學(xué)意義(P<0.05)。入院第1、3天CRP與APACHEⅡ評分無相關(guān)性(r1=0.148,P1=0.389;r3=0.104,P3=0.546);入院第1天CRP與預(yù)后無相關(guān)性(r1=-0.056,P1=0.747),入院第3天CRP與預(yù)后呈正相關(guān)(r3=0.364,P3=0.029);入院時PaCO2與預(yù)后無相關(guān)性(r=-0.322,P=0.056)。以CRP和APACHEⅡ評分作受試者工作特征(ROC)曲線,曲線下面積(AUC)分別為0.512、0.368。 結(jié)論:CRP的動態(tài)變化對判斷機械通氣的AECOPD患者的病情嚴重程度和預(yù)后有一定價值。

慢性阻塞性肺疾病急性發(fā)作期(acute exacerbation of chronic obstructive pulmonary disease,AECOPD)是重癥監(jiān)護病房(ICU)中患者發(fā)生呼吸衰竭的主要病因之一,感染是其發(fā)作的主要原因。C反應(yīng)蛋白(C-reactive protein,CRP)是一種急性時相蛋白,其作為炎性反應(yīng)的敏感標志物,在感染后2 h即升高,48 h達到峰值,半衰期短于24 h;一旦感染得到控制,CRP水平迅速下降[1]。本文探討了動態(tài)監(jiān)測CRP在行機械通氣的AECOPD患者中的臨床價值。
1資料與方法
1.1一般資料選取2012年1月—2013年12月在我院ICU行機械通氣治療的AECOPD患者36例,其中男性26例,女性10例;年齡58~91歲,平均(74.33±8.00)歲。AECOPD患者的診斷按照我國《慢性阻塞性肺疾病診治指南》2007修訂版中的診斷標準[2],同時排除肺部其他疾病以及其他系統(tǒng)疾病。
1.2方法分別檢測入院第1天、第3天的CRP,記錄入院時的動脈血氣分析結(jié)果,根據(jù)急性生理學(xué)及慢性健康狀況評分系統(tǒng)Ⅱ(acute physiology and chronic health evaluationⅡ,APACHEⅡ)進行評分。隨訪患者的28 d生存狀況,并據(jù)此分為生存組和病死組,比較兩組的CRP、入院時PaCO2和APACHEⅡ評分的變化。
2結(jié)果
2.1分組及比較入院28 d時,36例患者中27例存活(生存組),9例病死(病死組)。兩組性別、年齡的差異無統(tǒng)計學(xué)意義(P>0.05)。兩組入院第1天CRP差異無統(tǒng)計學(xué)意義(P>0.05),第3天CRP差異有統(tǒng)計學(xué)意義(P<0.05)。兩組的APACHE Ⅱ評分差異無統(tǒng)計學(xué)意義(P>0.05)。兩組入院時PaCO2的差異有統(tǒng)計學(xué)意義(P<0.05)。見表1。

2.2各指標的相關(guān)性分析入院第1、3天CRP與APACHEⅡ評分無相關(guān)性(r1=0.148,P1=0.389;r3=0.104,P3=0.546),入院第1天CRP與預(yù)后無相關(guān)性(r1=-0.056,P1=0.747),入院第3天CRP與預(yù)后呈正相關(guān)(r3=0.364,P3=0.029)。入院時PaCO2與預(yù)后無相關(guān)性(r=-0.322,P=0.056)。
2.3ROC曲線繪制ROC曲線,評價CRP、APACHEⅡ評分對預(yù)后評估的價值,AUC分別為0.512、0.368。見圖1。

3討論
CRP是一種非特異性急性時相蛋白,主要由肝臟在細胞因子IL-6的作用下合成,外周淋巴細胞也可合成少量CRP。CRP具有激活補體、促進吞噬以及免疫調(diào)控作用。CRP在健康人血清中僅微量存在;當機體處于感染狀態(tài)時,CRP的升高遠遠早于外周血白細胞計數(shù)和體溫的升高[3]。CRP的測定不受抗生素、免疫抑制劑、腎上腺皮質(zhì)激素的影響,且操作方便、價格低廉、重復(fù)性好[4]。
本研究中,入院第1天時生存組和病死組的CRP差異無統(tǒng)計學(xué)意義,而第3天時兩組CRP的差異有統(tǒng)計學(xué)意義。入院第1天時的CRP與預(yù)后無相關(guān)性,而入院第3天時的CRP與預(yù)后呈正相關(guān)。因此,CRP下降越快,炎性反應(yīng)控制越好,患者生存率越高。CRP持續(xù)高水平提示病情危重。CRP的動態(tài)變化對判斷行機械通氣的AECOPD患者的病情嚴重程度和預(yù)后有一定價值。
兩組入院時PaCO2差異有統(tǒng)計學(xué)意義,提示入院時PaCO2與患者的預(yù)后可能有關(guān),但經(jīng)Pearson相關(guān)分析后發(fā)現(xiàn)兩者無相關(guān)性(P=0.056)。這可能與本研究中病例數(shù)較少有關(guān),有待擴大樣本量進行進一步研究。
APACHE Ⅱ評分是目前ICU中應(yīng)用最廣泛的危重病評估系統(tǒng),可量化患者疾病的危重程度,評估患者的預(yù)后。APACHE Ⅱ評分與患者病死率直接相關(guān)。本研究中CRP與APACHE Ⅱ評分無相關(guān)性,但CRP、APACHE Ⅱ評分與預(yù)后的ROC曲線表明,CRP的AUC明顯大于APACHE Ⅱ評分,故CRP對預(yù)后的評估價值優(yōu)于APACHE Ⅱ評分。

Clinical Value of Dynamic Monitoring of C Reactive Protein in Patients with AECOPD under Mechanical Ventilation

AbstractObjective:To investigate the clinical value of dynamic monitoring of C reactive protein (CRP)in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD) under mechanical ventilation. Methods:The CRP levels were determined in 36 patients with AECOPD under mechanical ventilation at day 1 and day 3 after admission. The results of arterial blood gas analysis on admission were recorded. Acute physiology and chronic health evaluationⅡ(APACHEⅡ) was used for scoring. Patients were divided into survival group and death group according to the prognosis, and then compared with each other. Results: No statistically significant difference was found in age and sex between the two groups(P>0.05). There was no statistically significant difference in CRP at day1 between the two groups(P>0.05), while there was at day3(P<0.05) . No statistically significant difference was found in APACHE Ⅱ scores between the two groups(P>0.05). There was statistically significant difference in PaCO2between the two groups on admission. There was no correlation between CRP and APACHEⅡ scores(r1=0.148,P1=0.389;r3=0.104,P3=0.546) at day 1 and day 3 after admission. CRP at day 1 was not correlated with the prognosis(r1=-0.056,P1=0.747).There were positive correlation between CRP at day 3 and the prognosis(r3=0.364,P3=0.029). There was no correlation between PaCO2on admission and the prognosis(r=-0.322,P=0.056).The areas of CRP and APACHE Ⅱ scores under the receiver operating characteristic(ROC) curve were 0.512 and 0.368. Conclusions: Dynamic change of CRP contributes certain value to determining severity of illness and prognosis in patients with AECOPD under mechanical ventilation.
